Bluewater Metabolic & Clinical Services

One care team, each person in the right role.

Two providers lead your care: a full-time Nurse Practitioner and a part-time Registered Social Worker. Our nurses start your care with the intake visit, and a dedicated nurse runs each program alongside them.

Who does what

NP
Nurse Practitioner · full timeLeads your care. Assesses, diagnoses, prescribes, orders and interprets tests, and sets and reviews the plan for every program.
RSW
Registered Social Worker · part timeCounselling and psychotherapy, psychosocial and geriatric assessment, caregiver and grief support, and help finding community services. Appointments are offered on set days each week.
RN
RPN
Registered Nurses and Registered Practical Nurses: initial intakeYour first visit is with a nurse. They take your health history, medication list, measurements and screening questionnaires, so your time with the Nurse Practitioner or social worker goes to decisions and treatment rather than paperwork.
RN
RPN
Program nursesA dedicated RN or RPN runs each program day to day: metabolic health and weight, ADHD, men’s and women’s health, injections and infusions, and occupational and student health. They handle monitoring, teaching, treatments and follow-up visits, working with the Nurse Practitioner and the social worker on one shared plan.
Admin
Administrative teamBooking, forms, reminders, fees and receipts. Admin staff never make clinical decisions; anything clinical goes straight to a nurse or provider.

How your care runs

Nurse intakeHistory, medications, measurements and screening with an RN or RPN.
Provider visitAssessment and plan with the Nurse Practitioner, or counselling with the social worker.
Program follow-upYour program nurse keeps things on track between provider visits.
Shared reviewThe NP adjusts the plan; the social worker joins where it helps.
Everyone works within their own scope. Nurses act on Nurse Practitioner orders or approved medical directives, the social worker practises under the Ontario College of Social Workers and Social Service Workers, and the whole team works from the same chart.

Who you will see

You can check any clinician’s registration on the public register of the College of Nurses of Ontario or the Ontario College of Social Workers and Social Service Workers.
