Care across life stages

Older Adult Health & Medication Support

Personalized support for older adults and their caregivers, including medication reconciliation, assessment of medication benefits and risks, appropriate deprescribing and psychosocial assessment.

With your consent, our team works with your family, pharmacy and other care providers to develop a coordinated plan.

A thoughtful approach to medication

A comprehensive medication review includes reconciliation and a written plan. Deprescribing considers whether medicines can safely be reduced or stopped; changes depend on assessment and monitoring.

Psychosocial support can address mood, isolation, daily functioning and caregiver strain. Cognitive screening is distinct from a comprehensive dementia or capacity assessment.

Older-adult care fees

All ages · from $50

Brief Assessment & Minor Illness Care

Focused assessment for one straightforward concern, diagnosis where possible, advice and a prescription if clinically appropriate.

Fees start at $50 and may vary with complexity. Our team confirms suitability and cost before booking. Private fees apply only where permitted and after checking public coverage eligibility; prescriptions are not guaranteed.

Bluewater Metabolic & Clinical Services is not an emergency department or urgent-care service. For emergencies, call 911 or attend the nearest emergency department.
