Registered Social Worker services can address psychosocial needs that often do not require an NP appointment.

Services

  • Geriatric psychosocial support and care planning.
  • Caregiver and family counselling.
  • Grief, bereavement and loss.
  • Adjustment to illness, disability or major life transitions.
  • Stress and anxiety counselling within RSW scope.
  • Community-resource navigation and supportive planning.
  • Cognitive screening such as MoCA where the RSW is appropriately trained and the result is used within a defined clinical pathway.
A MoCA score is a screening result and does not independently diagnose dementia. Nurse Practitioner assessment may be recommended based on the clinical context.

Palliative care, comfort and family support

Psychosocial support may help individuals, families and caregivers navigating serious illness, changing care needs, grief and loss. Counselling and social work can form part of a coordinated palliative or home-based care plan alongside appropriate clinical and nursing services. Palliative and home-based care

Students and young adults

Comprehensive psychosocial assessment explores emotional wellbeing, school or work functioning, relationships, coping, safety and support needs. A first visit of about two hours (a 30-minute nurse intake, then a 90-minute RSW assessment) includes screening and a concise written support plan. This is not psychoeducational testing or a guarantee of accommodation approval.

Older-adult psychosocial assessment

Explore mood, coping, social isolation, daily functioning, caregiver strain, safety concerns and community supports with an RSW. A first visit of about two hours (a 30-minute nurse intake, then a 90-minute RSW assessment) includes a support plan. Medical or cognitive concerns may need a separate clinical assessment.
